The cholesterol & Heart Check
  1. The free Cholesterol and Heart Check (the "C&H Check") involves testing your cholesterol levels, measuring your blood pressure and reviewing your lifestyle. For full details, go to www.lloydspharmacy.com and select 'Cholesterol and Heart Check'.
  2. Before your C&H Check, you will be asked to sign a consent form and fill in a short questionnaire. A trained member of the Lloydspharmacy team will discuss your answers to ensure the test is right for you. A member of staff will explain the service to you and ask for some information regarding your medical history and lifestyle.
  3. In order that Lloydspharmacy might conduct the test, you will be required to provide a finger prick sample of blood (taken by Lloydspharmacy at the time of the test), permit your blood pressure to be taken and supply Lloydspharmacy with your personal details.
  4. The C&H Check should take approximately 10-15 minutes.
  5. You will get your results immediately. Dependent on your results you will be offered diet and lifestyle advice to help achieve a healthy heart. If appropriate, you may be referred to your doctor.


Terms and Conditions:
  1. In order to claim the C&H Check, you must purchase two promotional packs from the Flora pro.activ range, from any participating retailer. Promotional packs may not be available for all products in the Flora pro.activ range.
  2. Your free C&H Check service is provided by Lloydspharmacy Limited ("Lloydspharmacy").
  3. Your C&H Check will only be carried out by Lloydspharmacy on production of valid till receipt(s) at the time of your C&H Check, showing your purchase of two promotional packs from the Flora pro.activ range bought between 18.02.2008 and 31.05.2008.
  4. Closing Date: your free C&H Check must be booked and taken before 1st August 2008.
  5. Photocopied, damaged or illegible till receipts will not be accepted. Redeemed till receipts will be stamped by Lloyspharmacy and won't be valid to use again.
  6. The C&H Check is only available to UK residents aged 18 and over.
  7. Lloydspharmacy reserves the right to refuse to provide the service.
  8. The C&H Check is not appropriate for everyone, e.g. pregnant or breast-feeding women. To check you are eligible, contact Lloydspharmacy.
  9. Only one free C&H Check per person.
  10. The C&H Check is not available in all Lloydspharmacy stores. To find your nearest participating Lloydspharmacy, use the store locator at www.lloydspharmacy.com/storelocator and select 'Cholesterol and Heart Check' or alternatively, call 0800 389 2578.
  11. You can book your C&H Check directly at the store or over the telephone. Appointments are subject to availability in each participating store. You must be prepared to consider an alternative date if your preferred date is unavailable.
  12. No cash alternative will be offered.
  13. Lloydspharmacy reserves the right to amend the contents of the C&H Check at any point and without notice.
  14. The Promoter accepts no responsibility for the C&H Check or for any actions you take as a result of or in connection with the same.
  15. The Promoter shall not be responsible in the event that Lloydspharmacy refuses to provide you with a C&H Check.
  16. Lloydspharmacy accepts no responsibility for the distribution or availability of the promotional packs of Flora pro.activ.
  17. By taking part in this promotion, you will be deemed to have consented to the transfer of your personal data to the Promoter for the purposes of administering this promotion and any other purposes to which you have consented.
  18. These terms and conditions shall be governed by English law and the courts of England and Wales
